How we do it?
HOW WE DO IT ?
STAGE 1 >> ENGAGEMENT
Our Engagement Manager will help you understand how we can support you in achieving your NDIS objectives. We will work with you to assess your needs and determine the best course of action. A service agreement will be made during this meeting, outlining our collaborative process.
STAGE 2 >> PLANNING
Together with your circle of supporters, we will create a personalized plan that reflects your specific requirements, preferences, and goals. This will be used to tailor your services and pair you with the best support personnel. We will also complete a one-page profile to help us understand your needs and how to best meet them.
STAGE 3 >> EXECUTION
At Scientific Healthcare Services, we understand that implementation is key to success in navigating the NDIS. Your Engagement Manager will work with you to discuss how your goals are being achieved. We will link and coordinate paid, mainstream, and informal services in a service delivery setting to ensure that we can accompany you on your NDIS journey and support your efforts to "make it happen."
STAGE 4 >> MONITORING
Following the establishment of your supports and services, we will work together to keep an eye on your needs, wants, and goals. When you evaluate your plan with the NDIS, you will have access to accurate information since we will maintain track of how goals are being met. Typically, this occurs only once a year. scientific healthcare want to build and increase your skills, capacity, and confidence as you engage with us to take charge of your NDIS plan. You have the option to autonomously administer your own plan if you feel confident organizing your NDIS plan.
